You can make the rough draft and can edit/modify it later with ongoing time. You can take help from the employees as surveying and giving them the forms to fill so that you can collect the accurate report and do your analysis. You can take help from Google as what and how to make questions for that specific survey. Many free templates are also available online.
It will be good that you take much required info from the employees as you can ask them what duties they perform and in how much time they usually complete their work. Ask them what are the regular/daily duties of them what they have to complete in 1 week.
You also can observe the employees as how they do their work, how they actually interact with the clients and also keep an eye on them to know the minor details.
